Informed Consent for At-Home IV Therapy, Energy Therapy & Wellness Services
 

By booking a service with Dra Drip, I acknowledge and agree to the following:
 

1. Voluntary Participation
 

I understand that IV therapy, energy therapy, and wellness services provided by Dra Drip are elective and voluntary. I am choosing to receive these services at my own request.
 

2. Nature of Services
 

I understand that:

• IV Therapy involves the administration of vitamins, minerals, amino acids, NAD+ and other nutrients directly into the bloodstream.

• Energy Therapy is a non-medical therapeutic technique focused on fascia activation and emotional release.

• Surf Ritual is a wellness experience, not a medical service.

• These services are not a replacement for medical diagnosis, treatment, or emergency care.
 

3. Health Disclosure
 

I confirm that I have fully disclosed:

• All medical conditions

• Allergies

• Medications

• Chronic illnesses (including liver, kidney, autoimmune, cardiac, or metabolic conditions)

• Recent surgeries or hospitalizations

• Pregnancy or breastfeeding

I understand that withholding information may increase the risk of adverse reactions and that Dra Drip is not responsible for complications arising from undisclosed health conditions.
 

4. Risks & Possible Side Effects
 

I understand the potential risks, including but not limited to:

• Bruising, pain, swelling, redness at the IV site

• Nausea, dizziness, low blood pressure

• Allergic reactions

• Inflammation or irritation of the vein

• Temporary emotional release during Energy Therapy (crying, shaking, laughing, tension release)

I acknowledge that serious complications are rare but possible.
 

5. At-Home Safety
 

I confirm that the chosen location (home, Airbnb, hotel, villa) is safe, clean, and appropriate for treatment.

Dra Drip reserves the right to refuse service if the environment is unsafe or unsanitary.
 

6. No Guarantees
 

I understand that results may vary and that no specific outcome is guaranteed.
 

7. Right to Refuse Service
 

Dra Drip may decline or postpone treatment if:

• Vital signs are unstable

• I appear unwell

• I present contraindications

• Safety concerns arise
 

8. Refund if Service Cannot Be Performed
 

If Dra Drip determines that I cannot safely receive a service due to a medical contraindication, I am entitled to a full refund for that appointment.

9. Agreement

By checking the boxes in the booking form, I acknowledge that you have read, understood, and agree to this informed consent.
